Myopia Management: You Are in the Driver Seat

This course will review the mechanisms behind the development and evolution of myopia. It will also determine how we can influence eye growth from this knowledge. Analogy will be made to explain this phenomenon, comparing the retina as a driver travelling with 2 passengers (hyperopic and myopic defocus). Depending on whose passenger is the most vocal, the driver will turn to the right (hyperopic-elongation) or to the left (myopic-resistance to grow).When both signals are at equilibrium, the retina stops growing and emmetropization mechanism is restored.

What Evidence-Based Literature Does Not Tell You: Myopia Management from the Trenches
Dr. Thomas Aller, OD, FBCLA
In this course, attendees will learn about myopia progression and the various effective treatments now available, with an emphasis on the US market. Dr. Aller has over 30 years of experience in the clinical management of excessive myopia progression using all available tools. While that experience included extensive involvement in clinical trials, product development and regulatory approvals processes, it mostly consisted of treating individual patients with progressing myopia desiring to maximally slow that progression. In the usually accepted hierarchy of clinical evidence, randomized controlled clinical trials are only superseded by meta-analyses of such studies, but practitioners, who want to provide the best care possible for patients presenting to their offices wanting the most effective treatments, must be thoroughly knowledgeable about the relevant clinical trials in the field. But to best serve their patients, they also have to keep their ‘inner scientist’ on a bit of a leash. From a review of those studies, along with a number of case reports and retrospective analyses, Dr. Aller will seek to give the audience the benefit of some of his 30 years of experience to help provide the most effective treatment for each patient.

Myopia Case Studies

This grand round lecture will review the theoretical concepts and apply them to real patients. In this way, it becomes clear that myopia can only be managed one child at a time.

Safety of Contact Lenses Used for Myopia Management

The increasing use of soft multifocal and orthokeratology contact lenses for myopia management necessitates understanding of the safety of these modalities. An explanation of each modality and their effectiveness, both alone and in combination with low dose atropine, is presented in this lecture. The safety of each modality, including the incidence of sight-threatening complications are presented as well.
